Diabetes is a condition that requires constant attention and management. Our diabetes services include inpatient diabetes case management, an outpatient educational program for people with diabetes and their families, a diabetes foot care program and a gestational diabetes program. Those diagnosed with diabetes can receive nutritional counseling, personalized self-management guidance, and follow-up care.
In addition, our HealThy Self program features cardiac, pulmonary and vascular fitness to improve overall health and manage diabetes. Upon referral by a physician, clients meet with specially-trained nurses, exercise physiologists, dietitians and therapists. This care team designs a fitness program and individualized lifestyle plan that fits each person's personal needs. The team helps each client reach their individual goals in order to reach optimal health.
Most insurance companies, including Medicare, cover diabetes education costs. A physician referral is required to participate in the diabetes programs, and the Diabetes Center maintains contact with the patient's physician to keep them informed of the patient's progress. gestational diabetes program
For women who are diagnosed with gestational diabetes during pregnancy, we offer nutritional counseling, education and follow-up care. Certified diabetes educators work closely with patients and their obstetricians to create treatment plans, and send progress reports to referring physicians.
Pre-pregnancy counseling also is available for women with diabetes planning to become pregnant, as well as those who have had gestational diabestes with a previous pregnancy. Ask your physician for more information about the gestational diabetes program.
